Chapter 1Hallux Valgus My mother had hallux valgus (bunion) of her right foot since the age of fifty.Her big toe was bending towards her second toe and overlapped it.It was probably painful but she never allowed us to help her.She had special shoes to wear to fit her foot.It is also possible when she was small her mother forced her to bind her foot so that it look small and dainty as was the case in many Chinese women around her time.However I believed that it was most genetic because at the age of forty my elder brother also had hallux valgus and had an operation to correct it.My second brother and myself do not have the hallux valgus.What is Hallux Valgus?Hallux Valgus (Bunion) is the medical disorder that cause deformity of the big toe metatarsopharyngeal (MTP) joint in which there is medial enlargement and lateral deviation of the big toe.This is normally linked with bony and connective tissue enlargement of the medial aspect of the first metatarsal head that is called the bunion.A hallus valgus (bunion) forms when the big toe points toward the second toe.This produces a bump to occur on the inside edge of the toe.The source is lateral deviation of the great toe so as to put a valgus deformity on the first metatarsophalangeal (MTP) joint.A deviation of 15-20° is regarded abnormal.This deviation affects the biomechanics of the foot.It may induce subluxation of the first MTP joint and the big toe may even cover the second toe.Lateral subluxation induces a prominence on the metatarsal head (bunion) often followed by the formation of a fluid-filled bursa.This becomes painful as it scrapes against the shoe.Shoe wear does not cause hallux valgus.What are the causes of Hallux Valgus?PathophysiologyThe correction of the biomechanical factors may avoid excessive pronation and progression of the deformity:1. When walking, the hallux and digits stay parallel to the long axis of the foot.This is true normally regardless of how pronated or abducted the forefoot is.2. The pull of the conjoined adductor tendon, extensor hallucis longus, and flexor hallucis longus tendons makes sure that the hallux and digits remain parallel.3. Displacement of the joint gives the tendons mechanical benefit and this pushes the joint further.As this happens, the tension is produced on the medial aspect of the joint (with compression laterally).4. Medial tension induces ligaments to pull and cause the bone to proliferate on the dorsomedial aspect of the first metatarsal head.4. Lateral tension induces the sesamoid apparatus to stick in a dislocated position laterally.5. Remodeling happens laterally and medially and this involves joint cartilage.Hallux valgus can happen as a result of:1.
